Please note that new men’s officers (Local or District Directors, Secretaries, Board members etc) should request an induction via their local or district pastor by completing and returning this form to the Men’s Secretary / Treasurer.

National Men’s Ministries – Monthly Reporting Forms
Local & District Churches are asked to report monthly on their Men’s Ministries activities. Your reports should be sent to the National Men’s Ministries Secretary / Treasurer by the 5th of each calendar month.

Reports sent should be sent fully completed.
